For Overseas Filipino Workers:

2 Valid IDs, COEC (Consularized), Contract of Employment, Passport, Remittances/Bank Statement, SPA, Birth/Marriage Certificate, 2 pcs 2x2 and 3pcs 1x1,  Proof of Billing Adress, TIN

For Self-Employed:
2 Valid IDs, Business Registration (DTI/SEC), Mayor's Permit, ITR, Audited Fiancial Statements,
